Prior to filling out a referral form, please ensure you meet the following criteria.

 

You or your child is a resident of one of the following counties:
o Atchison
o Douglas
o Johnson
o Leavenworth
o Wyandotte

You have at least one child under the age of 24

You identify as a father or father figure to the child

You are 18 years or older
